Surgeons Discuss Management of Phyllodes Tumors

Debate Over Margins

The optimal negative margin width for phyllodes tumors is yet to be defined. Recent studies have shown that a margin of 0.1 mm is not inferior to a margin of 1 cm, suggesting that the larger margin is not needed for most patients with phyllodes tumors. Despite this, the National Comprehensive Cancer Network’s current guidelines recommend a 1-cm margin.

Given that the risk for recurrence is so low, Dr. Grange does not remove phyllodes tumors with a margin. After diagnosis by core needle biopsy, she enucleates the tumors, which allows her to confirm a diagnosis of benign phyllodes tumor. This approach also allows the breast tissue to reclaim its space and restores the breast to its original appearance.

Phyllodes tumors (PTs), which are rare and mostly benign in nature, are fibroepithelial tumors of the breasts. They account for less than 1% of all breast tumors and display a wide range of biological behaviors, ranging from benign to highly aggressive. Women between the ages of 40 to 60 years old are predominantly affected. However, cases in men, albeit rare, may also be seen, especially men affected by gynecomastia, an enlargement of the male breast due to hormonal imbalance
